The yearly New England Regional Nurse Practitioner Conference (ERNPC), hosted by the Massachusetts Coalition of Nurse Practitioners in Westborough, MA, focused this year on connecting practitioners, providers, and researchers with the insights and takeaways that would best support them in delivering care to vulnerable populations.

Waymark’s Clinical Care Model Specialist Melissa Jacobs was in attendance, sharing how the Waymark Care Model and Quality Teams integrate community health workers (CHWs), clinical pharmacists, and pharmacy technicians into structured chronic disease management and care delivery workflows. During her poster presentation, she successfully connected with care providers across various specialties and discussed with them the importance of structured health reviews in reducing acute care utilization.

In addition to sharing this research, Melissa was delighted to connect with nurse practitioners, researchers, and providers who remain committed to delivering high-quality care in the face of the access challenges patients consistently encounter. The sessions she attended offered actionable insights on strategies to address pediatric vaccine hesitancy through population-level interventions, translating research into real-time care delivery, and leveraging AI tools to support clinical decision-making.

It was incredibly energizing to see so many healthcare professionals all actively committed to learning from one another. Thank you, MCNP, for putting on such a successful event!
