We'll visit you at home, at a coffee shop, or wherever works for you. Together, we'll talk about what you want to achieve with your health.

Your care team will check in regularly and help with appointments, medications, resources, and more. You can reach us by phone, text, or in person.

Moving forward at your own pace

When you feel ready to manage your health journey independently, your care team will help you transition. And if your needs change, we’re always here to step back in.

What is Waymark?

Waymark is a free service for eligible patients from your insurance. We help you get to doctor appointments, manage your medications, find mental health support, and connect to community resources like food or housing assistance.

Does Waymark replace my doctor or PCP?

No. Waymark works with your doctor to help you stay healthy. We make sure you get to appointments, understand your treatment plan, and have what you need between visits.

Is Waymark really free?

Yes. If you have Medicaid through one of our partner health plans, Waymark is completely free. There are no costs or fees for Waymark services.

How do I get started with Waymark?

Call us at (415) 212-8993 or fill out our contact form. We’ll check if you’re eligible and connect you with our local care team. You can also ask your doctor to refer you.

How long will I work with Waymark?

As long as you need us. Some people work with Waymark for a few months, others for longer. You can stop anytime, and if you are eligible, you can always come back if you need help again.

What if my insurance changes?

If your insurance changes, your eligibility might change too. Let us know right away and we’ll help you figure out what’s next. We want to make sure you stay connected to the care you need.

I’m a provider. Can I refer a patient to Waymark?

Yes. You can refer patients by filling out our referral form or calling (415) 212-8993. We’ll reach out to your patient within 24-48 hours to get started.
