How do I know if my patient is eligible for Waymark?

Waymark serves patients enrolled in Medicaid through our partner health plans. Contact us and we'll verify eligibility for your patient. If they're eligible and interested in services, our local care team will begin working with them.If the patient isn’t eligible, we’ll refer them to resources that might help and let you or the referring provider know that we connected.

How does Waymark coordinate with my practice?

Waymark works alongside you to address barriers that prevent patients from following your care plan. We coordinate care activities with you and your treatment plan remains central to the patient's health management. Our care coordinators serve as your dedicated point of contact.

Does Waymark replace my role as the primary care provider?

No. Waymark extends your care team into the community to address social and behavioral health needs that are hard for you to manage within clinic visits. You remain the primary care provider and clinical decision-maker.
